HER-2/neu is a proto-oncogene located on the long arm of chromosome 17 encoding a 185-kd transmembrane tyrosine kinase growth factor receptor closely related to epidermal growth factor receptor. HER-2 expression stimulates growth and cellular motility and its overexpression is seen in 10% to 35% of human breast cancers that is associated with amplification of the gene in more than 96% of cases. HER-2 protein overexpression is associated with a poorer outcome for women with node-positive and node-negative lesions and might be associated with reduced or even adverse response to antihormonal therapy. It also might be associated with resistance to certain types of adjuvant chemotherapy and an enhanced response to others. Treatment with monoclonal antibodies against HER-2 might increase the response rate in combination with chemotherapy, and the time to disease progression and survival of patients with metastatic disease might be prolonged. Thus, in addition to being a prognostic indicator, HER-2 overexpression and amplification are important in determining treatment strategies. Furthermore, because the degree of overexpression and amplification is associated with survival and response to treatment, accurate, reproducible quantification is essential. HER2 is quantified relative to the control genes, Gastrin and IF2 (eukaryotic translation initiation factor). In some cases HER-2 pseudo-amplification created by chromosome 17 polysomy, therefore we use of Gastrin control gene for determination of degree of polysomy 17. Also IF2, is used since it is located near the centromere of chromosome 2, which is an area that is not deleted or amplified in breast cancer. The quantitative method calculates ratio of HER-2 amplification Vs Gastrin and IF2 genes.
1- In Her2/Gastrin, a ratio of 2 or more is considered positive (amplified), and less than 2 is considered negative (unamplfied).
2- A Her2/IF2 ratio above 2.2 is Her2 positive and a ratio of 1.1 + 0.35 is normal.
As positive control, we use Her2 amplified and Her2 unamplified DNAs.
The results of methods 1 and 2 must be comparable.
